Assistance with Daily Living helps you manage everyday routines so you can live as independently and safely as possible at home and in your community.[web:132][web:135][web:138][web:140]
ADL supports are tailored to your goals and capacity, and can increase or decrease as your independence builds over time.[web:132][web:135][web:140]
Social & Community Participation supports you to get out, connect with others and build confidence in real‑world settings, not just at home.[web:133][web:136][web:139][web:145]
Activities are co‑designed with you, based on your NDIS goals, interests and what is available in your local community.[web:133][web:136][web:141][web:145]
SLES is designed to support young people with disability in the first years after school, helping them build skills and confidence for work, further study or training.[web:127][web:129][web:130][web:134]
We work closely with school leavers, families, schools and other services to create a step‑by‑step transition plan over time, not just a one‑off intervention.[web:129][web:130][web:137]
Carers and families are central to good outcomes. Our carer education focuses on practical knowledge and realistic strategies that make day‑to‑day caring more sustainable.[web:132][web:135]
Education can be provided one‑to‑one with carers or alongside the participant, depending on what works best for your family.
Aevitas Care welcomes referrals from support coordinators, allied health professionals, plan managers, schools and community organisations working with NDIS participants who are self‑managed or plan‑managed.
