NDIS Support – ADL, SCCP & SLES

We support self-managed and plan-managed NDIS participants across Melbourne metro and regional Victoria. If you or your family member manage NDIS funding yourselves or through a plan manager, you can choose Aevitas Care as your provider.
No. Aevitas Care is a non-registered NDIS provider. Under NDIS rules, self-managed and plan-managed participants are allowed to use non‑registered providers, as long as supports are reasonable and necessary and fit within the plan.[web:186][web:189]
SCCP stands for Social & Community Participation. This funding helps you get support to join community, social and recreational activities, such as groups, hobbies, sport, classes, or events, so you can build skills, confidence and connections.[web:139][web:145][web:191][web:199]
At Aevitas Care, SCCP support might look like attending local activities with a support worker, learning to use public transport, joining clubs, or practising social skills in real‑world settings.
SLES stands for School Leaver Employment Supports. It is NDIS funding that helps young people in their final years of school, or just after leaving school, to build skills and confidence for work, further study or training.[web:186][web:189][web:195]
Supports can include work‑readiness training, work experience, travel training, life skills and help to explore realistic career pathways over a period of up to two years.[web:186][web:189][web:190]
We focus on three key areas: Assistance with Daily Living (ADL), Social & Community Participation (SCCP), School Leaver Employment Support (SLES), plus carer education. All supports are delivered in homes and communities, not in a clinic.
We are a mobile provider covering Melbourne metro and a range of regional areas across Victoria (including the western suburbs, Geelong, Ballarat, Bendigo and parts of Gippsland). You can see a detailed list on our Locations page.
If you have funding for Core Supports (Assistance with Daily Life or Social & Community Participation) or Capacity Building supports such as Finding and Keeping a Job (for SLES), it may be used with us as a non‑registered provider when you are self‑managed or plan‑managed.[web:139][web:188][web:191][web:199]
