Aevitas Care is committed to protecting your privacy and handling your personal information in line with the Australian Privacy Principles (APPs) under the Privacy Act 1988 and relevant NDIS privacy requirements.[web:200][web:202][web:211]

We provide support services to self-managed and plan-managed NDIS participants in Victoria. While we are not an NDIS‑registered provider, we aim to meet the privacy expectations set out in the NDIS Code of Conduct and Practice Standards, including respecting your dignity, confidentiality and choice.[web:201][web:203]

1. What personal information we collect

We only collect information that is reasonably necessary to provide our services and meet our legal obligations.[web:202][web:205]

• Identity and contact details – name, address, phone, email
• Date of birth, preferred name, language and communication preferences
• NDIS details – NDIS number (if provided), plan goals, funding categories, management type
• Support needs, daily routines and relevant health or disability information needed to deliver safe support
• Emergency contact, family or carer details (with consent)
• Information you or your representatives give us in calls, emails, forms or meetings

2. How we collect information

We may collect information:

• Directly from you (phone, email, website forms, assessment meetings)
• From your family, carer, support coordinator or other providers, when you have given permission
• From documents you provide, such as reports or plans

3. How we use your information

Your personal information is used to:

• Plan and deliver safe, person‑centred supports
• Communicate with you, your nominees and relevant professionals
• Manage bookings, invoices and administration
• Meet our legal, duty‑of‑care and record‑keeping obligations

We will not use or disclose your information for unrelated purposes (such as marketing) without your consent, unless required or authorised by law.[web:200][web:202]

4. Sharing your information

We may share information:

• With people you nominate – family, carers, advocates or plan managers
• With other providers (e.g. support coordinators, allied health professionals) involved in your supports, when you agree
• With government agencies or regulators if required by law or to prevent serious harm

When possible, we will explain what information is being shared, why, and with whom, and seek your consent first.[web:202][web:203]

5. How we store and protect information

We take reasonable steps to keep your information secure, including:[web:202][web:205][web:211]

• Password‑protected devices and secure cloud storage
• Limiting access to records to staff who need the information to do their job
• Training workers in privacy, confidentiality and secure handling of records
• Secure disposal or de‑identification of records when they are no longer required and after any legal retention periods

6. Accessing and correcting your information

You can request access to the personal information we hold about you and ask us to correct anything that is inaccurate, out of date or incomplete.[web:210]

In rare cases we may need to refuse access (for example, if it would seriously impact someone else’s privacy or safety). If this happens, we will explain why and discuss options with you.[web:210]

7. Website, forms and cookies

When you use our website or online forms, we may collect basic technical information (such as browser type, pages visited and time on site) to help improve our services. This information is usually de‑identified and not linked to your name.

If you submit a contact form, the details you provide (name, contact details, message and role) are used only to respond to your enquiry or process a referral.
